Yes. Transitional Care Management, Remote Patient Monitoring, and Chronic Care Management are typically covered by Medicare and many Medicare Advantage plans, Medicaid and Commercial Insurers when medical necessity criteria are met. Standard copays or coinsurance may apply depending on your plan.

No. NaviCare works collaboratively with existing primary care provider and specialists. Working as part of a patient's healthcare team, we strengthen communication and coordination — we do not replace patient's ongoing medical providers.
Yes. Our services are primarily telehealth-focused for convenience and rapid access for patients anywhere in Nevada. In-clinic or in-home visits may be arranged when clinically appropriate.
